What We Value in Health: A Coalition Vision for Better Care in India
GDI-incubated Leapfrog to Value (L2V) advances value-based care to improve health outcomes and reduce costs in India, South Africa, Kenya, Ghana, and beyond. L2V supported major national milestones, including India’s first value-based care policy, Ghana’s groundbreaking payer-provider contract, and South Africa’s public commitment to value-based care. Building on a critical mass of innovation in value-based care by multiple healthcare actors, L2V has now pivoted to convening locally-led coalitions of stakeholders— from patient advocates to health financing institutions— to develop country-specific strategies in India and Kenya. In early 2025, India’s leading health system thinkers came together to form India VBC coalition. Together they have created: What We Value in Health: A Coalition Vision for Better Care in India, a strategy to realign how India invests in health, moving from volume-based to value-based care.
India’s health system is expanding coverage rapidly. But without deliberate shifts in how we measure, finance, and deliver care, expansion risks entrenching inefficiency rather than improving outcomes. This strategy identifies the root causes, fragmented care, opaque cost and outcome data, misaligned incentives, and proposes four catalytic initiatives to address them.
The coalition, comprising leaders from Dvara Holdings, ACCESS Health International, Aditya Birla Health Insurance, Narayana One Health, Indian School of Business, and the Clinton Health Access Initiative and others, facilitated by Leapfrog to Value, has co-designed a practical roadmap with four transformational initiatives for systems change: a People’s Commission for Health Improvement, a Primary Health Care Design Laboratory, a Business Case for Quality and Safety, and pilots of Coordinated Care Bundles for high-burden diseases and surgeries.
This is India’s agenda for equitable, outcome-focused health investment—grounded in evidence and ready for action.
